Post void residual urine in female12/30/2023 ![]() ![]() Although long-term consequences of PUR are rarely reported and small studies showed negligible if any clinical impact on long-term urogynecological disorders, only a few data are available on the potential long-term micturition problems of increased PVR after vaginal delivery. PUR can lead to denervation, detrusor atony, and bladder dysfunction if it is not recognized in time. Trauma to the pelvic floor muscles, the detrusor muscle itself, and overdistention of nerve fibers might impair bladder sensitivity, cause periurethral obstructing edema, and hormonal changes may influence bladder function as well. Levator ani muscle avulsion seems to be associated with persistent postpartum voiding dysfunction, but the etiology of PUR is thought to be multifactorial. Several theories have been suggested, including physiological, neurological, and mechanical processes during pregnancy and delivery. The pathophysiology of PUR is poorly understood. The incidence of PUR ranges from 0.18 to 47% depending on the varying definitions and the time interval of follow-up used. Overt PUR has been defined as the inability to void within 6 h of delivery or after removal of the catheter, whereas covert PUR means an increased post-void residual urine volume (PVR) of more than 150 ml after spontaneous micturition. Postpartum urinary retention (PUR) is a serious and frequent complication after childbirth. ![]() Obstetric awareness, early active management, and developing management strategies in the postpartum period might preclude lower urinary tract morbidity. In most cases PUR resolves early, but voiding difficulties persist more often than previously thought, and for these patients the consequences are devastating. Eighty-nine percent of the patients with PUR had operative vaginal deliveries, emphasizing the importance of this risk factor for PUR. Quantile regression did not reveal any factor contributing to earlier recovery. Multichannel urodynamics confirmed in all patients with persisting retention an acontractile detrusor and de novo stress urinary incontinence in 4 cases. ![]() Long-term voiding disorders were found in 8.2%, 6.7%, and 4.9% after 1, 2, and 3 years respectively. If retention persisted longer than the lactation period, multichannel urodynamics was performed. PVR was measured sonographically every 2 days until day 15, then after 6, 12, 24 and 36 months and, if increased, the patients were instructed to perform clean intermittent self-catheterization. In our tertiary referral urogynecology unit in the University Women’s Hospital of Bern, Switzerland, all patients who were referred for PUR were asked to participate in this study. Pathophysiology, risk factors and management of PUR are reviewed. The aim of this study was to assess long-term persistence of elevated PVR (post-void residual urine) volume after PUR. The paucity of data on long-term outcome after PUR contributes to these drawbacks. The incidence ranges from 0.18 to 14.6%, but the importance of prompt diagnosis and appropriate management is often underappreciated. Postpartum urinary retention (PUR) may cause long-term urogenital tract morbidity. ![]()

For years the vehicles were left to the elements.Ībout 20 years later, the studio decided the ECTO cars would be refurbished, so they were taken to the shop that originally constructed the cars, Cinema Vehicles Services in North Hollywood. In 2010, the promo car was put on the Barrett-Jackson auction block in Arizona, where it sold for $80,000.Īfter being used for promotional purposes (including lending ECTO-1A to Universal Studios, Florida, where tourists regularly swiped items from the car and it was smashed into by a park vehicle), the ECTOS ended up on the Sony back lot. Diy medieval costumes12/29/2023 ![]() Maybe we will go to a Renaissance fair this summer before he grows out of the armor.Hear ye, hear ye! Throw it back, and we mean way back, this Halloween with these regal Renaissance costume ideas. He especially liked standing perfectly still in a crowd and then moving suddenly to startle onlookers. It takes a good 10 minutes to dress my son (I was the squire), but he loved the armor and had a blast wearing it at all of our Halloween events. We also spray painted a knit shirt silver to replicate chain mail, although it ended up not showing very much. My son’s sword was made of wood (just a lot of cutting and sanding) and finished similarly to the armor. A couple hundred rivets and eyelets and several feet of Velcro later it was all fully assembled. I know, Velcro isn’t very medieval, but with 12 different pieces of armor to put on, leather buckles would be way too slow. Velcro, either self-adhesive or riveted on, was used for all the rest of the attachment and cinch points. I contact cemented the finger armor directly to a pair of work gloves for the gauntlets. I used metal eyelets and leather cord to assemble the plates that hung from the breastplate and for the laces that attached the sabatons or foot armor to my son’s tennis shoes. ![]() As I mentioned, I used leather craft rivets for all of the pivot joints. I used gold metallic paint instead of silver to make the emblems on the breastplate, shield, and gauntlets stand out.Īfter painting, it was time for assembling. At this point the armor looked kind of plain and fake (Tin-Man like), so I created a faux patina by lathering it with a water based black acrylic paint and then buffing it back to silver until I got the right effect. Everything was then painted with metallic silver spray paint. I used gray Plasti-Dip to seal the foam side of pieces that would be showing. All of the polystyrene surfaces got a layer of gray spray primer. Once the costume was constructed and fully test fit, it was time for dis-assembly and painting. There were over 100 different pieces to cut, laminate, drill, and test fit (not to mention my fair share of mistakes and redos). Later, after painting, I would use proper leathercraft rivets. Holes were drilled at each pivot joint, and then I test fit the pieces again using the brads for the pivots, which could be easily removed prior to finishing. I really could only make curves in one direction (no compound curves), but that was enough for the costume. For pieces that needed to be curved (most of them), I used a heat gun to heat and then shape the foam-polystyrene construction over a curved surface. ![]() The foam cutout was then laminated to a grossly cut sheet of polystyrene using contact cement (Dap Weldwood Original), and then the polystyrene was trimmed to shape with a heavy duty scissors. I used 0.5” exercise floor mat foam for the shield since it needed to be sturdier. Smaller pieces or pieces that had to pivot together used 2mm foam (“foamies” sheets from a craft store). For the large body pieces I used 0.25” thick foam (breastplate, helmet, large arm and leg pieces). Luckily, we only had to do templates for one side of the body, as you can just flip the templates over for the other side.Īfter we liked how a prototyped piece worked, the card stock template was used to cut out EVA foam. This process took a while (many prototypes) to get the right size and mobility of each piece. Using card stock and metal brads for joints, we cut and fitted all the various pieces to my son’s body. I didn’t have a pattern, so we had to make it as we went. The next step was figuring out the shape and size of all the pieces. After receiving my order of EVA foam and 0.020” thick polystyrene sheet, I prototyped a simple piece of laminated armor and decided it was a go. I wanted to try it and see what we could do. However, I remembered seeing a technique for making light-weight armor by Amethyst Angel in which she uses thin polystyrene plastic sheet laminated onto EVA foam. I have used EVA foam on past costumes, which you can heat up and shape, but I was worried how it would hold up for armor pieces. Clearly steel was out of the question for cost, tools needed, and weight issues. Next was figuring out which materials to use. Scouring the web for images and recalling suits I’ve seen in museums, my son and I settled on an idea pretty quickly.
